A Case of Mixed Dependence with Caffeine and Ephedrine

Abstract
Caffeine and ephedrine, both are popular CNS stimulants, have been widely used as a component of antipyretics, antitussives, drinks, and many other street drugs without formal prescription. It is thought that there are a number of unreported cases who abuse such drugs. But systematic epidemiological studis or case reports on caffeine and ephedrine dependence are rare at the moment. We experienced a case of 36-year-old woman with chronic overuse of Panpyrin¢ç, Bacchus ??, and coffee which contain ephedrine and caffeine. She exhibited severe withdrawal symptoms including headache, nausea, vomiting, insomnia, agitation, hallucinations, and craving, and was improved by gradual tapering of the drugs and conservative treatment after 2 weeks. It is estimated that she has consumed caffeine 2500mg and ephedrine 450mg a day. We report herein a case of mixed addiction with caffeine and ephedrine, and reviewed the previous reports and literatures. The potential risk of addiction of caffeine and ephedrine in the general population was also discussed.
